Athletic Physicals Available in May 2009
Any student wishing to participate in athletics at any Hays CISD middle or high school must obtain an athletic physical prior to the start of the school year. All physicals should be submitted to the athletic training office of Hays or Lehman High School by July 27, 2009. (Please bring the physicals to your respective feeder high school – do not return any physicals to a middle school.) This date applies for all sports; spring and fall alike. Please turn in physicals as early as possible to make sure they are filed and at the appropriate school by the start of athletics.
Hays CISD, in accordance with the University Interscholastic League, requires a current athletic physical to be completed before any participation in athletic practices, classes or competitions in-season or off-season. All athletic physicals must be completed on the 2009- 2010 green physical form (available after May 1st). No other form or note will be accepted for participation in athletic activities.
The Hays CISD Athletic Training Departments host athletic physicals every year for only $15.00. Local healthcare providers will be on campus during four days in May to provide this convenient service to you! An immunization clinic will also be offered during some of the physical times (see other side). Please note the dates and times listed below for physicals. To schedule an appointment, please call the athletic training offices after April 27th. This requirement can be fulfilled by your family physician. Please note that even if your family physician performs an athletic physical, it must still be on the green UIL / Hays CISD 2009 – 2010 form.

May 12th from 3:00 – 6:00 p.m. (MS students)
May 19th from 3:30 – 6:00 p.m.
Lehman High School – Gymnasium
May 14th from 3:00 – 6:00 p.m.

Our school nurses will be offering the Hep A, Hep B, MMR, Tdap, Polio,and Menactra Vaccines. Please make sure to bring a copy of your current shot records, signed parental consent (if under 18 years of age) and cash for payment. Forms are available for parental consent from the nurse’s clinic. The cost will be $5.00 per child, regardless of the number of immunizations needed. Please contact your school nurse or the Hays High School athletic trainers with any further questions.
